Life-saving measures transform Bénin’s healthcare landscape

The Bénin’s healthcare system has entered a transformative phase where financial barriers no longer obstruct urgent medical intervention. Authorities have formally implemented the policy of immediate coverage for life-threatening emergencies, with a clear directive: treat now, settle the bill later. At the forefront of this initiative stand two flagship medical institutions in Cotonou: the Centre National Hospitalier Universitaire Hubert Koutoukou Maga (CNHU-HKM) and the Hôpital de la Mère et de l’Enfant Lagune (HOMEL).

A full-scale emergency response

Previously, accessing emergency care often required advance payment of a co-pay or acquisition of medical kits by distressed relatives. That requirement is now obsolete.

On the ground, the shift is palpable:

  • Ready-to-use medical supplies: Emergency pharmacies at CNHU-HKM and HOMEL have been fully stocked. Essential drugs, consumables, and resuscitation kits are immediately available to medical staff without prior financial clearance.
  • Around-the-clock staffing: Medical and paramedical teams operate in shifts to ensure rapid triage and seamless care upon patient arrival, whether via ambulance or self-admission.

« Our overriding goal is to stabilize patients within minutes of admission. Financial or administrative considerations come only after their vital prognosis is secured, » explains an emergency physician at CNHU.

Streamlined care pathways eliminate delays

To ensure this policy translates into real efficiency, the care process follows a strict, linear protocol designed to eliminate unnecessary waiting times.

The journey begins with immediate admission and triage. A qualified staff member conducts a rapid assessment to determine if the case qualifies as an absolute emergency.

Once identified, the patient is moved directly to medical care. At this stage, treatment and medication are provided upfront—no pre-payment is demanded—with the sole focus on stabilizing vital functions.

After the crisis is resolved, the post-emergency phase begins. This is when billing occurs retroactively. If needed, patients are connected with social services to address financial obligations in a compassionate and tailored manner.

Sustaining the new model remains a challenge

While widely praised as a historic humanitarian and social milestone, the initiative places significant demands on hospital administrations—particularly in managing supplies and recovering costs later. The government is banking on public responsibility and targeted subsidies to ensure the long-term feasibility of this upfront-free care model.

By removing the ‘pay-at-the-window’ culture during life-or-death situations, Bénin takes a decisive step toward universal health coverage, placing human dignity and the right to life at the heart of its health policy.
